[postgis] Documentation Updates

I am going to try and re-do the documentation for the next update, with
some re-organization to keep things comprehensible for people. The top
two items for me are:

- Fully separate "programmers" documentation and "users" documentation.
- Increase the number of usage examples dramatically.

Lower priority items are:

- Some integrated documentation on the JDBC stuff, with usage examples.
- Some documentation on the Shape reader/writer.

I am open to other suggestions,
